SERVICES OFFERED

What types of care do you provide?
Elevated Primary Care offers a full spectrum of services designed to support your health across all stages of life. Our care model includes:

  • Preventive exams & annual physicals

  • Lab testing & health screenings

  • Chronic condition management (e.g., diabetes, hypertension, thyroid disease)

  • Hormone therapy and sexual health for men and women

  • Medically guided weight loss & performance support

  • Adolescent care (typically ages 13+)

Do you offer weight loss support or hormone therapy?
Yes. We offer personalized, evidence-based care for both. This may include lab testing, lifestyle coaching, nutritional strategies, and medication when appropriate. Our programs are designed to improve energy, strength, and long-term metabolic health.

Do you treat complex or chronic conditions?
Absolutely. We take time to evaluate and manage overlapping concerns—such as autoimmune symptoms, fatigue, cycle changes, GI issues, and more. If you’ve been told “everything is normal” but don’t feel well, we dig deeper.

Do you treat anything else?
Yes! Elevated is more than just a traditional primary care clinic. We frequently help patients with:

  • Hormonal imbalances & mood shifts

  • Skin issues, hair changes, or sleep problems

  • Sexual dysfunction or discomfort

  • Mental health concerns (in partnership with trusted therapists)

We’re here for anything that doesn’t feel right—even if no one else has found an answer. We listen carefully, connect the dots, and create care plans tailored to your body and your goals.

INSURANCE & BILLING

Do you accept insurance?
We are currently out-of-network with insurance but plan to join select plans in the future. In the meantime, we offer affordable self-pay pricing and provide superbills for reimbursement.

Texas Law Update:
Texas now requires insurers to count your cash payments toward your deductible if you submit a receipt—meaning you may hit your deductible faster when paying directly for care.

Can I use my insurance for labs or prescriptions?
Yes. Many lab partners and pharmacies we work with accept insurance—even if your visit is self-pay.

How much does a visit cost?
Pricing varies by service, but we always share rates upfront—no surprise billing.

SPECIALTY QUESTIONS

Do you offer Botox or aesthetic treatments?
Aesthetic services including Botox and filler are coming soon. Stay tuned for updates.

Can you refer me to a specialist if needed?
Yes. We regularly coordinate specialty referrals and collaborate with local specialists when your care requires it.

Do you see children?
We typically see ages 13+, or younger girls who have started menstruating. We do not currently offer pediatric vaccines or well-child visits.

TECHNOLOGY & SUPPORT

Do you offer telehealth?
Yes. Most initial visits are in-person, but many follow-ups can be done virtually when appropriate.

Is there a patient portal?
Yes. Our portal gives you secure access to lab results, appointment scheduling, and direct messaging with your provider.

What if I need help after hours?
While we do not offer 24/7 on-call coverage, you can message us through the portal anytime. For urgent after-hours care, please visit urgent care or call 911.
